Annual wellness visit and initial preventative physical examination
The ABC's of annual wellness visit and IPPE
- IPPE( Initial Preventative Physical Examination) Covered Once in a lifetime
- Must be done no later than 12 months after the effective date of the first Medicare Part B Coverage period.
- Record and Evaluate your medical history : Past , Family, Social review of Current conditions Allergies Immunizations and Medication review
- Health Risk Assessment Form : Assessment of functional Ability /fall Risk/ cognitive Impairment , Depression Screening
- Make sure you are up to date with preventative screenings and services , such as cancer screenings and shots
- Advance Care planning
- Exam: Height, Weight, BP, BMI Visual Acuity Not a Head to Toe Exam
- Education , Counseling or referral for any abnormal findings
- Baseline EKG if needed
- AAA Ultrasound screening (if meets guidelines)
- Deductible does not apply however the IPPE does not include any clinical laboratory tests, any prescriptions and management of any new or old medical problem. It focuses on health promotion and disease prevention and detection to help beneficiaries stay well.
AWV (Annual Wellness Visit)
- Annual Wellness Visit (may not be done within first 12 months of coverage)(May not have had IPPE within last 12 months)
- Deductible does not apply however the AWV does not include any clinical laboratory tests, any prescriptions, and management of any new or old medical problem. It focuses on health promotion and disease prevention and detection to help beneficiaries stay well.
- Record and Evaluate your medical history : Past , Family, Social review of Current conditions Allergies Immunizations and Medication review Include History of Current Providers / Suppliers and family
- Exam: Height, Weight, BP, BMI Visual Acuity Not a Head to Toe Exam
- Health Risk Assessment Form : Assessment of functional Ability /fall Risk Assessments , Cognitive Impairment , Depression Screening
- Creation of Screening Checklist A written Plan for Future Screenings and Health management
- Education , Counseling or referral for any abnormal findings
Things that are not covered, as part of your “AWV or IPPE” and will require copays and deductibles:
- Prescriptions
- Any Laboratory other than cancer screenings
- Management of any new or old medical problems.
These items require your physician to document a separate note detailing complaints and findings not included and are coded separately from the AWV and IPPE.
